Subject: WITHDRAWAL CARDS
Effective immediately, Teamsters Local #77 will honor requests for withdrawal
cards
when employment is interrupted because of resignation, illness, work injury,
or leaves for any other valid reason.
Below is a brief explanation which we hope will help you to better understand
the
procedural requirements:
1. It is every member's personal responsibility to obtain his/her
own withdrawl card.
2. Union dues must be paid for the current month in which an
employee resigns, or is on leave of absence, or leaves his/her
employment for any other reason, before a withdrawal card can be
issued.
3. Remember, if a withdrawal card is not obtained at the time
an employee resigns, or is on leave of absence, ALL delinquent
Union Dues must be paid from the date of separation until the
date an employee is returned to work.
4. Fifty cents ($.50) check or money order only, NO CASH along with the member's name, home address,
and place of employment must be forwarded to Local #77, BEFORE THE END OF THE MONTH in which a member is laid off, resigns, on
leave of absence, or leaves his/her employment for any reason.
5. Provided the member's Union Dues are in good standing as
specified in #2 above, immediately upon receipt of the Fifty
Cents ($.50), a Withdrawal Card will be processed and forwarded
to his/her home.
